I was given a prescription for Chloroquine for Malaria prevention.

The instructions on the vial said take two tablets daily instead of two tabs weekly.

The tabs were 250mg.

I knew this was wrong because I am an RN, but if I had taken them I am told I would have been extremely ill or worse. I would have been in Belize in a remote area.

I understand this error should have been reported to Head office by the pharmacist.

I would like to be assurred that it was. I only want to know what checks you have, or need to add to avoid errors in transcribing the order from the script to the bottle.
