I received a letter stating that there was a consumers reward program lottery held on jan. 5, 2009 and my name was attached to a ticket number 31579130 that drew the lucky number of 7, 11, 23, 31, and 33 and i had ultimately won in the second category. along with a letter was a check in the amount of 4, 900. The letter stated that i was selected randomly through computer from consumer database directory system, and that i needed to call my agents Noel Johnson or April Harris at 905-598-1154 as soon as i received the notification to finalize my claim
