Our intake form allows you to report detailed data about you and your families health history. On-Site Blood Panel and Urinalysis For your convenience our skilled phlebotomist will perform your blood work in a casual and relaxed environment. The fasting blood work will be based upon your age, gender and risk factor assessment.
