I filled out a free membership form from a ballot box and now they keep sending me bills every month

I filled out a free membership form from a ballot box in a restaurant, i never did work out, and on top of that they send me a bill every month..this has been over a year now... i cant owe anything and i never signed any contract... as well they have no bank acct number or form of payment... but still they are trying to bill me..I am NOT a paying member... but this paperwork they send says i owe them money!!!.but they continue to hassle me by mail... i have called the police...

i have called the club many times... they say sorry.. will take my name out of computer.but never do... the general manager is convenietly never around... i need help to get this settled... any one know what i can do?
