Unauthorized Credit Card Charge

I have had a charge lodged to my credit card labeled as "elitepack". I dusputed this hcarge with my Credit Card company as I did not authorize this. Coincidentally this has happened after ordering a Free Trial from Dazzle Smile and also one from Purely White. I suspect this is tied in with Dazzle Smile as I canceled my Purely White Membership a coule of weeks ago but didn't cancel Dazzle Smile until today when I realized they were charging me about $90/month.
